میری جان پاکستان
محبت کی زباں میرا وطن ہے
وفا کی داستاں میرا وطن ہے
زندگی تو جیسے بھی ہوگزر جاتی ہے، سورج آگ برسا رہا ہو،موسم سرما کی یخ بستہ ہوائیں شدت کی سردی کا سبب بن رہی ہوں ، موسم بہار میں گلستان کے گلہائے رنگارنگ خوبصورتی بکھیر رہے ہوں۔ ہرلمحہ ہر آن زندگی کاہمامحو پرواز رہتا ہے، لیکن وہ شخص قابل صد افتخار ہے جو زندگی کی بو قلو موینوں سے محبت و پیار سے نبردآزما ہوتا ہے، اور محبت و پیارکی پتنگیںاُس وقت تک نہیں چڑھائی جا سکیں گی جب تک ماحول خوشگوار نہ ہو اور انسیت بھری فضاء نہ ہو۔
اللہ تعالیٰ کا لاکھ لاکھ شکر ہے کہ اللہ تعالیٰ نے ہمیں پاکستان کی صورت میں نعمت غیر متر قبہ سے نواز رکھا ہے۔ اس سے محبت ہمارانہ صرف طبعی تقاضا ہے کہ ہمارے عزیز واقارب یہاں رہائش پذیر ہیں ، ہماری سانسوں کی یہاں آمدورفت ہورہی ہے، اس کے شجر، اس کے حجر ہماری زندگی میں ایک اہم اور فعال کردار ادا کرتے ہیں۔ بلکہ اس سے محبت ہمارے ایمان کا حصہ ہے۔ فرمانِ رسالت مآب صلی اللہ علیہ و آلہٖ وسلم ہے کہ ’’ حب الوطن من الایمان‘‘ وطن سے محبت ایمان کا حصہ بھی ہے۔ ہمارے تو ایمان کی تکمیل ہی وطن سے محبت میں مضمر ہے۔ اگر محبت اور پیار ہے تو ایمان مکمل ہے، اگر وطن سے پیار اور محبت نہیں ہے تو دعویٰ ایمان حقیقت سے یکسر خالی ہے۔

Intake of Anti-Epileptic Drugs and their Influences on Sexual Dysfunctions

Epilepsy which is also called seizures disorder is an uncontrolled action of the central nervous system. It is not a single disease but a set of neurological disorders. Actually in this situation, the brain does not receive a precise signal and as a result an abnormal condition is produced that is usually involuntary in action. In this review, we aimed to focus on the relationship of anti-epileptic drugs with sexual dysfunction and adaptation of better remedies that improve a patient’s family life. Sexual dysfunction is a common comorbidity in people with epilepsy which badly affects their quality of life. Sexual dysfunction is caused by different factors like psychiatric problems, anti-epileptic drugs (AEDs) and social factors etc. Sexual dysfunctions include ejaculatory failure, lessen libido, penile erection in men and irregular menstrual cycle in women. Common drugs such as Topiramate, Gabapentin (GBP), Valproate (VA), Carbamazepine (CBZ), Olanzapine (OL) and Risperidone (RTG) that are in practice to treat epilepsy usually produced adverse effect on sexual dysfunction. Even though a lot of studies have been carried out to control sexual dysfunction in epilepsy’s patient, but still research is going on. Medicine such as Cyproheptadine, Mianserin, Buspirone, Yohimbine were found better to treat epilepsy with minimum side effects of sexual dysfunction. Moreover, it is also seen that certain vasodilators,  folate , and vitamin supplements are effective in improving the quality of life.

Cytochrome P450 Genetic Variants As Predictor of Personalized Therapy in Pakistani Population

The CYP2C9 and CYP2C19 polymorphism are associated with pretreatment drug response prediction, metabolism and disposition. Pakistan consists of a population comprising of various ethnic groups residing in different regions of the country each claiming diverse ethnic origins. The identification of the CYP450 genotypic composition of these populations is therefore necessary to avoid adverse drug reactions in these individuals. The study objective was to investigate the CYP2C9*2, CYP2C9*3, CYP2C19*2, CYP2C19*17 frequency in nine Pakistani ethnic groups and to check their association with hypertension. DNA was extracted and analyzed by AS-PCR. Four phenotypes were observed and they were: extensive metabolizer (EM), poor metabolizer (PM), intermediate metabolizer (IM) and ultra-rapid metabolizer (UM). Regarding population, over all the percentage of predicted PM allele (CYP2C19*2) was 29.0% compared to (CYP2C19*17) UM allele 23.70%. Among the studied groups, Saraiki and Brahui showed highest percentage of PM allele (40%, 36%) whereas Parsi and Hazara had highest percentage of UM allele (37% and 30% respectively). In case of CYP2C9, CYP2C9*2 showed highest frequency in Saraiki (51%) and Burusho (45%), while in case of CYP2C9*3 Brahui (49%) and Hazara (47%) showed highest frequency. Overall, 75%, 25%; 64.2%, 35.8 % of CYP2C19*2 and 66.6%, 33.4%; 75.6%, 24.4% of CYP2C19*17 wild type and mutant allele frequency was observed in patients and controls (p<0.05). Similarly, 51%, 49%; 76%, 24% of CYP2C9*2 and 46.5%, 53.5%; 46%, 54% of CYP2C9*3 wild type and mutant allele frequency was observed in patients and controls (p<0.05). In conclusion the high prevalance of CYP2C9*2, CYP2C9*3, CYP2C19*2 and *17 in Pakistani population leads to the recommendation of a pre-treatment test to monitor drug response and dosage (personalized medicine) to avoid post-treatment adverse drug reactions.
